Travel Montana Survey Forms

Travel Montana is now providing our Travel Planner/Web Site survey forms in .pdf format. We strongly encourage you to call our office at 406-841-2788 prior to filling out your survey to ensure the proper form is filled out and accompanying photos are in a useable format. Any photographs submitted to Travel Montana must be accompanied by the photo release form found below (limit 6 photos).

Lodging Facilities

Lodging Facilities are required to provide their Facility Tax ID Number (BED TAX COLLECTIONS) in order to be included in the State's annual Travel Planner and Electronic Marketing Projects. If you are unsure of your Facility Tax ID Number or if you have questions, please contact Jeff Salandi (jsalandi@state.mt.us) at the Montana Department of Revenue (406) 444-5421. If the number is not included on the form the form will be returned to the sender.

The first box of all lodging facilities contains the information for the state Travel Planner and must be filled out. The keywords are specific to the type of facility. To insure the correct keywords are being used make sure that the primary type of business is the same as the type of survey being filled out. Unsure? Call 406-841-2788 to find out. It is necessary to have a firm set date for an event to be included in the state promotional service. Events must be open to the public and of interest to tourist. Travel Montana reserves the right to initiate, accept or reject any event information that does not fall within the Travel Montana's mission statement. It is the responsibility of the event promoter to provide accurate details of the event, including date. Please sign any survey before returning. Any missing information may result in the survey being returned for resubmission.

The following events will not be included in the promotional services: Pancake breakfasts, community service projects (such as weed pulls, trash pick-up etc.), flea markets, rummage sales, gallery or museum exhibits/shows (museums will be given hot links to their own websites to promote exhibits). Ongoing weekly events such as concert series, summer theater activities will be listed by start and end dates with the description having the individual dates specified. Exceptions may be made for unique circumstances related to events listed above. Travel Montana cannot be held liable on any promotional errors for free listings of event information.
